全踝关节置换术(Salto?Talaris)
Overview（概述）
Thissurgicalprocedureisperformedtoremoveportionsoftheanklethatarediseasedorseverelyinjuredandtoimplantadevicethatwillhelpmanagepainandrestoremobilitytotheankle.
这种外科手术是为了去除患病或严重受伤的部分踝关节，并植入一种有助于控制疼痛和恢复踝关节活动的装置。
Preparation（准备）
Inpreparationfortheprocedure,thepatientispositionedandanesthetized.Thesurgeoncreatesanincisioninthefrontoftheankletoaccessthejoint.
准备手术时，将病人摆妥体位并麻醉。外科医生在踝关节前方作切口以进入关节。
RemovingDamagedBone（去除死骨）
Aportionofthetibiaisremoved.Insomecases,partofthethefibulamayalsoberemovedtomakeroomfortheimplantdevice.Thetopofthetalusiscutawayandsmoothed.
将胫骨远端去除一部分。有些情况下，会去除部分腓骨为植入假体腾出空间。去除距骨的顶部并打磨。
InsertingtheImplant（植入假体）
Thebottomportionoftheimplantispositionedonthetalus.Thetopportionofthedeviceistheninsertedintothebaseofthetibia.Forsome,bonegraftisplacedinthelowerspacebetweenthetibiaandfibulatofusethebonesandinsurethatthedeviceremainssecurelyinplace.
将假体的底部安装在距骨上。然后将假体的顶部插入胫骨的远端。对于有些病例，会在远端胫骨和腓骨之间植骨以使骨融合，确保假体保持固定。
EndofProcedure（手术结束）
Theincisionisclosedwithsuturesorsurgicalstaples,andtheankleisbandagedandplacedinasplint.Ahospitalstayonetothreedaysafterthesurgerymayberequired.Thesplintisremovedseveraldaysaftersurgerytoallowmovementoftheanklefortherapyexercises.Fullweightistypicallynotappliedtothefootforatleastsixweeks.Ananklesupportmayberequiredforuptooneyearafterthesurgery.
手术完成后，用缝合线或外科皮钉关闭切口，包扎踝关节并用夹板固定。手术后可能需要住院1~3天。手术后几天去除夹板，允许脚踝运动进行治疗练习。6周内患足不能完全负重。手术后1年内可能需要保护踝关节。
Followup（随访）
Onceananklehasbeensuccessfullyreplaced,thepatientwilllikelyhavetofollowupwiththesurgeononanoccasionalbasistomonitortheimplant.Thepolyethelene-basedmobilebearingcanwearoutovertimeandmayneedtobereplacedtoensuretheimplantcontinuestofunctionproperlyformanyyears.
成功进行踝关节置换后，患者应定期访问外科医生以观察假体。聚乙烯做成的活动性轴承可能会随着时间的推移而磨损，可能需要更换，以确保假体持续多年的正常运行。
